What consents do I need to submit with my human interest story?

• Each person appearing in, who is heard in, or who is talked about in, a
  human interest story video, photo, or recording (as applicable) must complete
  and sign the “Authorization, Consent, and Release for Use of Name, Voice and
  Likeness.”
• Each person who took or made the picture, video, or recording must complete
  and sign the “Photography/Video Release.”
