National Core Indicator’s Surveys

NLACRC encourages you to participate and complete the National Core Indicator’s surveys. Your participation helps your regional center and DDS understand how services and supports impact you, your family, and loved ones. Data collection will end on June 30, 2024. Surveys are mailed directly to you from the State Council on Developmental Disabilities (SCDD). (Fill out the survey on paper and mail it in postage is already paid) OR Use the code in your letter to fill out the survey online.
