Let DDS know how we’re doing!

The Department of Developmental Services (DDS) has a new survey about how NLACRC supported you during your IPP meeting.

If you recently completed an IPP meeting in 2026, please complete the survey below!

The survey is quick, anonymous, and helps DDS know how you are being supported by NLACRC.
