Reminder! NLACRC is operating and available to consumers, families, and providers! We can be reached at each of our three (3) business offices (our office phone numbers are listed here) Monday through Friday, from 8:30 a.m. to 5 p.m. Please remember that our physical offices continue to be closed to the public until further notice. Delivery of paper documents or correspondence: To protect the well-being and safety of the people we serve, as a result of COVID-19 NLACRC is only accepting the delivery of all documents and communication by email, fax, and US mail delivery at all three office locations (email and fax preferred if possible to mitigate the spread of COVID-19). During this time, courier, Fed-Express, and UPS deliveries will only be accepted at our San Fernando Valley office. Click here for our office locations.

SPECIAL INCIDENT REPORTING
Alternate special incident reporting measures have been implemented to report COVID-19 related incidents. IT IS IMPERATIVE THAT YOU READ THE INFORMATION BELOW TO BECOME FAMILIAR WITH THESE ADDITIONAL MEASURES. To report COVID-19 related incidents, complete a special incident report and report to hotline@nlacrc.org.
The Department of Developmental Services (“DDS”) has recently amended reporting requirements for COVID-19 related special incident reports. The department is asking that all providers and regional centers submit special incident reports for events that meet the following criteria:
- An individual who tests positive for COVID-19;
- An individual receives medical attention at a hospital, emergency room, or urgent care clinic due to COVID-19 symptoms; or
- An individual’s death is related to COVID-19, either by confirmed COVID-19 positive testing or by medical diagnosis unconfirmed by testing.
North Los Angeles County Regional Center (“NLACRC”) is seeking for providers to continue to submit special incident reports for COVID-19 events that meet these criteria to sir@nlacrc.org, hotline@nlacrc.org, and notify case management. To avoid multiple notifications, vendors may report potential/direct exposures and test result in one SIR, rather than separate reports.
In an effort to help ensure the health and safety of consumers and providers within our community, NLACRC will continue to track all incidents related to COVID-19, even if they do not meet DDS’ criteria. With that in mind, NLACRC is seeking for providers to continue to submit an incident report to SIR@nlacrc.org and case management for COVID-19 incidents that do not meet DDS’ amended criteria. Providers are no longer required to report COVID-19 incidents that do not meet DDS’ amended criteria referenced above to hotline@nlacrc.org.
