SPECIAL INCIDENT REPORTING

Alternate special incident reporting measures have been implemented to report COVID-19 related incidents.  IT IS IMPERATIVE  THAT YOU READ THE INFORMATION BELOW TO BECOME FAMILIAR WITH THESE ADDITIONAL MEASURES.  To report COVID-19 related incidents, complete a special incident report and report to hotline@nlacrc.org

If you become aware of any consumer that has tested positive or is symptomatic of the COVID-19, please immediately notify executive administration using hotline@nlacrc.org.

Healthcare providers should immediately notify their local or state health department in the event of an identification of a PUI for COVID-19. When working with your local or state health department, check their available hours.
