Reminder!  NLACRC is operating and available to consumers, families, and providers!  We can be reached at each of our three (3) business offices (our office phone numbers are listed here) Monday through Friday, 8:30 a.m. to 5 p.m.  Please remember that our physical offices continue to be closed to the public until further notice.  Stay safe and well!

SPECIAL INCIDENT REPORTING

Alternate special incident reporting measures have been implemented to report COVID-19 related incidents.  IT IS IMPERATIVE  THAT YOU READ THE INFORMATION BELOW TO BECOME FAMILIAR WITH THESE ADDITIONAL MEASURES.  To report COVID-19 related incidents, complete a special incident report and report to hotline@nlacrc.org

Additional Special incident reporting (“SIR”) requirements have been implemented to report COVID-19 related incidents.  Please report all COVID-19 related incidents as directed below.

If you become aware of any consumer that has tested positive for COVID-19, is in the process of testing for COVID-19, is symptomatic of COVID-19, or had direct contact with an individual that tested positive for COVID-19, please take the following actions:

Non-Residential Service Providers Reporting Requirements for COVID-19 related incidents

Residential Service Providers Reporting Requirements for COVID-19 related incidents

  1. Submit a SIR to sir@nlacrc.org (Click here for SIR form)
  2. Submit the SIR and notify NLACRC’s hotline email at hotline@nlacrc.org
  3. Report the COVID-19 incident to Los Angeles County DPH at 877-777-5799
         a. Provide your name (Caller)
         b. Provide your phone number (Caller)
         c. Provide your email address (Caller)
         d. Provide the city from where you are calling (Caller)
         e. Provide a brief description of the information, i.e my consumer is exhibiting signs
             and symptoms of COVID-19, my consumer had direct contact with an individual
             that tested positive for COVID-19. (Note: DO NOT disclose any consumer  
             protected health information under HIPAA (such as date of birth, medical 
             information, consumer contact information, etc.)

Healthcare providers should immediately notify their local or state health department in the event of an identification of a PUI for COVID-19. When working with your local or state health department, check their available hours.
