Dear Service Providers,

The Los Angeles County Department of Public Health has included regional center service providers in Phase 1A, Tier 2 of the Vaccination Distribution Plan and as such, your staff who provide direct services to individuals with a developmental disability are eligible for vaccination. 

• An appointment must be made at one of LA County’s Points of Dispensing through the county’s website.To schedule an appointment for vaccination, please go to http://publichealth.lacounty.gov/acd/ncorona2019/vaccine/HCWSignup/
• If your preferred location is not accepting appointments, please consider alternate locations that may have other options for availability.

The following items will be required at the time of the appointment:

1. A copy of this regional center letter (this may be printed or displayed digitally on a phone, tablet, etc.)
2. Photo ID
3. Proof of employment:
   1. Pay stub, employee ID, business card with employee name, or similar

      OR

   2. Employment verification letter from employer that is on letterhead and includes the employee’s name
