Service Planning
General Information
As your partners in the California
developmental disability service system, our service coordinators
and staff are trained to help you find services and opportunities
to help you live as independently as possible.
You and your planning team will choose services that are
right for you and your family using a Person-Centered Planning
Process. We provide some services directly, but most will
come from other agencies, organizations and businesses.
We will help you gain access to the services that we do
not provide directly.
Examples of the types of services we provide include: diagnostic
evaluations, lifelong case management, advocacy in securing
services from schools and other organizations, assistance
in locating and using community resources, community resource
development, genetic counseling, prevention programs, community
education and outreach, parent support services and program
monitoring, evaluation and technical assistance to service
providers.
Services and supports may be received
through:
Generic services
Government supports such as Medi-Cal, Medicare, Supplemental
Security Income (SSI) and senior services. Sometimes non-government
organizations such as Easter Seals Society, Ability First, family service agencies, and religious organizations
may be explored.
Natural supports
Includes personal relationships that enhance the quality
and security of life (such as family and friends).
Private supports
For example, family health insurance.
